What the assessment covers

A decision that can be explained, implemented and reviewed safely.

The review considers both the possible benefit of reducing falls from bed and foreseeable harms such as entrapment, climbing, increased fall height, restriction and incompatible equipment.

Person and consent

Mobility, cognition, communication, behaviour, body size, preferences, capacity considerations and previous falls or climbing.

Bed-system compatibility

Bed, mattress, overlays, rail, gaps, compression, rail height, attachments and manufacturer instructions considered together.

Alternatives and controls

Lower beds, positioning, sensor systems, floor mats, environmental changes and other proportionate options where appropriate.

Observation and measurement

Relevant configurations and potential entrapment areas are recorded; technical faults are referred to the equipment provider.

Care-plan instructions

Clear use, checking, escalation, cleaning/removal precautions and actions after any equipment or clinical change.

Review schedule

Named triggers and review frequency based on presentation, equipment changes, weight change, incidents and local policy.

Deliverable

A usable bed-rail and bed-system risk record.

The report sets out evidence, benefits, hazards, compatibility findings, alternatives considered, agreed controls, implementation actions and review triggers.
