Your contact details, the location and a brief description of the help needed. Clinical information is shared only through the agreed referral route, never in the booking form.

What is included in an assessment?

One visit and a written moving and handling plan. The fee is shown in the booking form before you confirm; travel, extended work or urgent input is agreed with you first.

How quickly can someone be seen?

Red priority within 24 to 72 hours, amber within two weeks, green within four weeks. Availability is confirmed with you before the visit.

Availability is confirmed with you before the visit. AmberPriority Within 2 weeks A package under review, a provider concern, a planned equipment change or a reablement window that needs a decision soon. Booked into the next available priority slots. GreenPlanned Within 4 weeks Scheduled reviews, training, audits and caseload work agreed in advance. Standard scheduling.
